How does a patient transition from hospital to nursing home?
After a hospitalization, a hospital social worker or case manager typically coordinates the nursing home transfer. A physician must write orders for the transfer and establish care at the nursing home. When patients are going to one of our affiliated facilities, Dr. Pintauro can be involved in this transition from the start.
